A CSV file (comma-separated values) is the required format for uploading patient data. The structure is critical to ensure the system processes the information accurately. The column headers in the file must include First Name, Surname, Health Number (10 digits), Birthdate, and E-Mail Address exactly as specified. The formatting rules are as follows:
- Email and Birthdate Requirements: Both fields are mandatory for patient information to be imported successfully.
- Date Format: Birthdates must follow the dd-mm-yyyy format.
- Age Restriction: Individuals over the age of 18 will not be transferred during the upload process.
- File Accuracy: Carefully review the CSV file for errors, such as missing or incorrect data, inactive patients, or deceased individuals.
It’s highly recommended that the office thoroughly audits the file before uploading to ensure all active patients are included and the data adheres to the formatting rules.
